Tributary Advisory Group was founded after years of clinical practice and health system leadership, with a more recent focus on care model design following completion of an MBA with a public policy emphasis. That shift sharpened the tension already visible in primary care: traditional fee-for-service models were increasingly strained, while fully concierge care, though effective for some, remained inaccessible for many patients and difficult for systems to scale responsibly.

Out of that tension, a single middle-ground membership-based primary care model was designed and launched within a major academic medical center. While only one model was implemented, the process revealed a broader spectrum of viable middle-ground approaches, each defined by different degrees of access, service intensity, and patient contribution.
